The surround 4.0 output on M-Audio Revolution 7.1 doesn't work.

This is because the internal buffers are wrongly assigned in the combination of
plug and route plugins.

This bug may affect other outputs using route plugins, or mmap modes.

The definition of surround40 config in ICE1724.conf is anyway buggy.
The slave channels must be 6.
