Due to a small bug in the PAM implementation shipped with OpenSSH, unexpected
death of either the monitor program leaves the PAM child around. See

for both a patch and some further details for the reproduction. The fix is
included in OpenSSH 4.2+

Closing old LATER+REMIND bugs as WONTFIX - if you still plan to work on it, feel free to reopen and set to ASSIGNED.

In case the report saw repeated reopen comments, it's due to bugzilla timing out on the huge request ;(
