This seems to happen when the root filesystem is either read-only (lots of read-only errors during boot) or when the root partition is mounted using a label (normal "green" boot). LABEL=root / xfs defaults 1 0 I also get this at the rescue system's login prompt. Something really is not right here. pam? Nothing in syslog. On a "normal" install where no labels are involved (xfs though), it happens right away after [install][reboot][postinstall] at the mingetty prompt. So, I have not changed anything.
