I cannot print to "Letter" size media in Inkscape

and "Print Direct".

It seems that inkscape produces output for A4 media
but the printer uses Letter and the user cannot switch
to Letter in inkscape.

It appears you do File->Document Preferences.  This is upstream's (strange) choice.
