The same problem is present in Yast2's Xen Source dialog. Screenshot attached.
As an aside, I don't see any provision for using an existing,
previously-installed partition (i.e., one already populated using "Install Into
Directory" - unless the absence of a Source is supposed to indicate this.

The table widget has been fixed in yast2-vm-2.12.3

I created a separate bug report for the second problem - see bug #113750
