Kommander Editor (also known as Dynamic Dialog Editor) icon is missing from the
KDE menu. (K button->Development->GUI designer)

Steps to reproduce: Install the software above from any BETA3 mirror,
  then look for this software in KDE menu.

I added an icon to the 3.5 branch in the KDE repository. Feel free to take 
from there.
