In "Add to panel"/"Applet" menu, "Binary clock", "Runaway process catcher" and
"Taskbar" are missing their icons.

Furthermore, in the "Remove from panel"/"Applet", the following are also missing
their icons: "System tray" and "Taskbar"

Also, In the Panel menu, "Help"/"Report bug" doesn't have an icon

The "Binary clock", "Runaway process catcher" and "Taskbar" icons were only
added upstream for KDE 3.5 release. KDE 3.4 doesn't contain them.

And also in KDE 3.5 no application has/uses an icon for "Report Bug..."
