Since last dist-upgrade "cups-pdf" delivers a pdf-file with a single blank page on every print to the CUPS-PDF printer.

Tumbleweed last upgrade     2021-06-08
cups-pdf version:           3.0.1-1.16
ghosscript version:         9.54.0-1.1

Changing either config-file "/etc/cups/cups-pdf.conf" or source-file  "cups-pdf.h" to remove the command line "-c .setpdfwrite" option did not help at all.

I can confirm this issue.
Also using Tumbleweed KDE
As far as I can tell it started when ghostscript moved to 9.54
I use the cups-pdf feature quite regularly as some of my apps don't have a pdf print feature built in.

Last time I printed and it worked as follows:

Is the recent OpenSuse Tumbleweed source tree for Cups-pdf available somewhere?

I solved the problem by mounting an older .iso of tumbleweed in to my repos and switching the ghostscript and ghostscript-X11 to the older versions
9.52-2.2

Now it works

So the problem lies there

In lack of an elder offline install image I added the repo:

> http://download.opensuse.org/distribution/leap/15.3/repo/oss/

did reset the cups-pdf setup and switched back to GS and GS-X11 Version 9.52-3.

No success with GS 9.52 - same negative result.
