Steps to reproduce:
mutt -nF /dev/null -v -d1

First output line is:
DEBUG was not defined during compilation.  Ignored.

Within output, in the Compile options section:
-DEBUG
Instead of +DEBUG

This was supposedly fixed here:
https://bugzilla.opensuse.org/show_bug.cgi?id=1203184
But I just tried mutt from OSS default repo, and problem still the same.
What did it mean in previous report that it was fixed? Was it only in the version of server:/mail repository?

Yes.

For Leap this package is inherited from SUSE:SLE-15:Update/mutt which is a different lineage. Maintainer has not put it in as a SLE update for this issue, and that seems to be the intend state.
