The upstream commit is 3d32e4db (kvm: fix excessive pages un-pinning in kvm_iommu_map error path).

For SLE 12 this fix came in under stable update 3.12.33.
For SLE 11 SP3 a patch based on upstream commit 3d32e4db was included.
